HELLO WolfPack PARENTS!

The county has adopted a new method of submitting required paperwork to participate in athletics.  You will be able to submit all paperwork electronically.

This is a requirement. 

I will need all parents to visit www.dragonflymax.com or download the app DRAGONFLYMAX

• Click on Login/Sign-Up in the right hand corner
• Click on Sign Up For Free
• Fill in your information
• Follow the prompts to create a PARENT account with your email and phone number.  It will send a code to your email as a confirmation to continue.
• Enter your child’s school code when prompted and confirm this is the correct school. 6MV32N6MV32N
• Click ADD A CHILD and them follow the prompts to create your child’s profile.   Please choose the sport your child currently participates in.  You can add more sports later.
• Click on THINGS TO DO
• Click on GET STARTED

There will be 4 things you need to complete. You will need your insurance information.

1.  Complete the medical history first.  There will be 6 tabs that need to be completed.  Please make sure you hit NEXT after each tab…NOT DELETE.  Click DONE when finished.  If you missed anything, it will tell you.

• Under GENERAL INFO – You will upload the last sheet of your child’s physical form with the doctor stamp.   Skip birth certificate.  Skip immunization.  

2.  Complete the Concussion Form.  You will simply have to electronically sign your and your child’s name.  Please put the date next to your name

3.  Complete the Sudden Cardiac Arrest Form.   Same as above

4.  Complete you Insurance Waiver.  The insurance information you filled in from your history form will automatically transfer.  Sign and date.

5. Complete the CCPS SUMMER 2020 RETURN TO PARTICIPATION Release of Liability Form & Guidelines Form. Sign and date

When you are done, it should tell you if you did not do something correctly.

THIS IS MANDATORY.   Athletes who do not have this completed by the beginning of the first practice will be suspended from participation.  PLEASE go ahead and get it over with.   If you need any assistance, please email Coach Belton or email your head coach.  I will be glad to help you.

Please also download the DragonFly MAX from the App Store of Google Play and sign-in.
